Was the incorrect parameter being monitored to detect failure? Was there insufficient time to detect an impending failure before the failures actually occurs?
Examples
Example 1
- Turbine bearing temperatures were being monitored to predict impending failures. However, failures occurred even though there was no prediction of failure based on temperature levels. Vibration should have been monitored instead because it was a better predictor of impending failures.
Example 2
- Megger testing was being performed yearly on motors in an attempt to predict failures of the windings. However, a number of failures occurred even when megger testing did not predict a failure. Infrared inspections were a better predictor of these types of motor failures.
Typical Recommendations
- Provide guidance on the typical parameters that can be monitored to predict failures for different types of components.
- Ensure that monitoring equipment for condition-based maintenance is appropriate for the component.
