Examples
Example 1
- Operators were trying to determine whether the maintenance had been completed on a feed pump. However, they had difficulty locating the work requests associated with the pump. As a result, there was a delay in the startup of the system.

- Personnel were having trouble printing documents over the network while they were trying to print out some batch sheets. The operators had made these same batches several times before, so they decided to proceed without them. As a result, an error was made while making up one of the batches and the product was ruined.
