David J. Grattan PE, CFSE has over 30 years of experience in process hazard analysis (PHA), layer of protection analysis (LOPA), safety instrumented systems (SISs), hazard and operability (HAZOP) analysis, failure modes and effects analysis (FMEA), control system HAZOP (CHAZOP) and human factors reliability analysis (HRA), all related to process safety management (PSM). He specializes in managing, leading, and documenting PHAs, hazard reviews, and LOPAs for OSHA/EPA compliance for a wide range of industries, including liquified natural gas (LNG), refining, petrochemical, chemical, plastics, and natural gas liquid (NGL) industries.
He also has an extensive background in functional safety (FS) implementing all phases of the SIS safety lifecycle, including design, specification, safety PLC programming, proof-test procedure writing, factory and site acceptance testing, as well as leading functional safety program audits at 7 US based refineries to identify gaps in implementation of the safety lifecycle.
